Home Visiting / Service Coordination

Purpose/Goals:

• Identify the mandated responsibilities of service coordinators in the Help Me Grow system
• Describe the service coordination tasks involved in the development, review and monitoring of the Individualized Family Service Plan (IFS)
• Describe the key components of ongoing home visits in the Help Me Grow system
• Explain how "culture" impacts home visiting
• Explain the importance of safety when conducting home visits

Who should attend?

Training requirement for all Clinical Supervisors of Service Coordinators, FIN Consultants, Family Support Specialists, Help Me Grow Project Directors, Home Visitors/Paraprofessionals, and Service Coordinators.

Continuing Education

Successful completion of the workshop includes attendance at the entire event and completion of the training evaluation. Akron Children's Hospital (OH-234 080107) is an approved provider of continuing nursing education by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center's Commission of Accreditation. OBN-001-91 (Contact Hours: 5.5) Social Work Hours: 5.5 and MRDD's Early Intervention or Case Management: 6 credits.

This workshop is sponsored through funding provided by the Bureau of Early Intervention Services (BEIS) of the Ohio Department of Health and is presented by the Family Child Learning Center.
